10-Year Full-Arch Warranty (working terms)

Coverage

  1. What is covered: the dental implants placed by Dr. Fisher and the final fixed zirconia full-arch prosthesis delivered by Avenue of Smiles, for ten (10) years from the date the final prosthesis is delivered.
  2. Implant failure: if an implant placed by Dr. Fisher fails to integrate or is lost during the warranty period, Avenue of Smiles will remove it and place a replacement implant at no charge for the surgical and implant fee. (Decision: whether grafting required for the replacement is included.)
  3. Prosthesis: if the final zirconia prosthesis fractures, chips, or debonds under normal use, Avenue of Smiles will repair or replace it at no charge, at the practice’s discretion. (Decision: repair vs. replace criteria; whether replacement after a set number of repairs is covered.)
  4. Components: abutments, screws, and attachments placed by the practice are covered against failure under normal use.
  5. Tiers: applies to the Plus and Premium tiers (zirconia final prosthesis). The Essential tier’s same-day acrylic teeth carry the 12-month warranty already stated on the landing page. (Decision: whether Essential’s implants carry the 10-year implant coverage even though the acrylic prosthesis does not.)

Patient obligations — the warranty is void if not met

  1. Post-operative visits: the patient must attend every scheduled post-operative check after surgery and after delivery of the final prosthesis. Failure to appear for a scheduled post-op visit voids the warranty.
  2. Recall visits: the patient must attend professional maintenance and examination visits at Avenue of Smiles at the interval Dr. Fisher prescribes (Decision: every 6 months, or as prescribed), including the annual prosthesis removal/inspection if prescribed. Missing a recall visit voids the warranty. (Decision: a grace period, e.g., a visit rescheduled within 60 days does not void.)
  3. Home care: daily hygiene as instructed, using the devices recommended by the practice (water flosser, brushes).
  4. Night guard: the patient must wear the occlusal guard provided by the practice nightly if one is prescribed; fractures in a patient who was prescribed a guard and did not wear it are not covered.
  5. Health disclosure: the patient must disclose changes in health, medications (including bisphosphonates and antiresorptives), and smoking or vaping status at each visit.

Exclusions

  1. Damage from accident, trauma, sports injury, or misuse (chewing ice, hard candy, bones, non-food objects).
  2. Failure related to smoking, vaping, or tobacco use during the warranty period. (Decision: exclude entirely, or cover with reduced terms.)
  3. Failure related to uncontrolled medical conditions (for example uncontrolled diabetes), new radiation therapy to the jaws, or new antiresorptive/bisphosphonate medication after placement.
  4. Untreated bruxism where the prescribed night guard is not worn (see 9).
  5. Work performed, adjusted, or repaired by any other dentist, laboratory, or third party.
  6. Normal wear, staining, or discoloration that does not affect function; cosmetic preference changes.
  7. Loss or damage of removable items provided (night guard, temporary prosthesis) after delivery.
  8. Costs outside the covered repair or replacement: sedation or anesthesia fees, additional grafting, travel, lost time, or treatment by other providers.

Terms

  1. Warranty period begins on the date of final prosthesis delivery and ends ten (10) years later.
  2. (Decision) Proration: full coverage for years 1–5; from year 6 to year 10 the patient pays a stated percentage of the then-current fee for replacement of the prosthesis. State plainly if adopted.
  3. Non-transferable; applies only to the patient treated.
  4. Coverage requires the account for the original treatment to be paid in full or current under its financing agreement.
  5. Claims: the patient must contact Avenue of Smiles and be examined at the practice before any repair; the practice determines whether the failure is covered under these terms.
  6. Remedy limited to repair or replacement as described; no cash refunds.
  7. The practice may adjust these terms for new patients prospectively; the terms given to a patient at delivery govern that patient’s warranty.
  8. Warranty terms are provided in writing at treatment acceptance and again at delivery, and are posted at avenueofsmiles.com/warranty.
